Wellington Catholic, in collaboration with Family Counselling and Support Services for Guelph Wellington, Developmental Services Ontario Central West Region, Passport Community Development Team and Upper Grand District School Board hosted Oh! The Places You’ll Go! a virtual discussion providing important information on the transition from school to adulthood for those with a developmental disability.
Intended for caregivers and students aged 15-21, the 30-minute virtual presentation discusses the school transition plan, making the transition from school to adult life, how and when to connect to adult developmental services, passport funding (community participation funding allocated to eligible individuals), and service coordination.
Throughout the presentation you will hear from each of the community partners about the transition from school life to adulthood, what a transition plan is, how to create a transition plan, building a calendar of a full life, the services and programs available after secondary school, how to get connected in the community and make friends, and exploring ways you can lead a fulfilling and happy life.
